There was a small tweak to the League Match Forms for the 2016/17 season, namely to add the name of the shot clock operator. Download here!

Each team in the local league is required to hand-in one half of a completed match form at the end of each game. The other half is so the club has its own record of the match’s details for entering on to Fixtures Live.
